The symbolism of it all that's so obviously tearing Dean apart - if Sammy kills Madison, in Sam's mind, Dean has to kill Sam. There's a lot of great fic out there about that ep and how ridiculous, in a way, it is for Sam to feel that way: big difference between Sam asking his brother to kill him and a girl Sam just met and screwed, however much of a connection they had, asking him to do it. I'll have to find some o' the fics for you...
